As a courtesy to our patients, Dr. Slowenko will accept assignment of dental insurance payments, with the understanding that our patients pay any portion of the dental fee not covered by insurance on the date of treatment. Dental insurance is a contract between the insurance company and the policy holder. However, our business team will still help you to obtain your maximum benefit whenever possible by sending a treatment preauthorization to your insurance company.

Financial Arrangements

Payment is expected when dental treatment is rendered. Our practice offers many payment options: Visa, Mastercard, American Express, debit, cheque, and cash.  If additional or alternate financial arrangements are required, arrangements may be made with our Office Manager.
